Blind masseur Zatoichi is nursed back to health by a young woman after he is shot by a gang member. Zatoichi, who had come to the village to repay a debt, now feels further indebted. He commits himself to use his amazing sword skills to help the young woman's father, whose river-crossing service is under attack by the same gang responsible for Zatoichi's wounds.

Blind swordsman/masseuse Zatoichi befriends a young woman returning home with her baby. When gangsters mistake her for Zatoichi and kill her, Zatoichi determines to escort the baby to its father. He gains the reluctant help of a young pick pocket and together they travel to find the baby's father. But they do not reckon on the father's reaction to their arrival, nor on their own growing feelings for the child.

In the early Edo period, among the pirate ships that roamed the seas near Ryukyu and Korea, there was a ship called the Sueyoshi, led by a man known as Misaki Shogoro, who was infamous for his devilish cruelty. Shogoro had a rule: he would kill all the men from the ships he raided, while the women would be distributed among the crew for their pleasure. However, he himself never touched any woman. Aboard the ship was a young girl named Kogane, who had been raised on the ship since she was a child and dressed as a boy. Unbeknownst to others, she had grown to secretly adore Shogoro.

While transporting a bride-to-be in a closed carriage (kago) while during a thunderstorm, a group of servants and onlookers suddenly come upon an apparition of a ghostly white-draped female figure floating down to the ground. As she turns to face the crowd, the woman reveals the disfigured right side of her face. As the procession stands in utter horror, the ghost then takes flight again and down a side street. However, someone surrounding her witnesses the moment Okiku breaks the plate on purpose.

Along the way, Sentaro saves a young woman named Okimi (Yuko Miki) from a bad womanizer and meets a Yakuza named Katsugoro (Yoshie Mizutani). He then visits Katsugoro boss Kanjiro Inariyama (Ganjiro Nakamura II) to train to be a yakuza, but to his surprise discovers that Katsugoro is Kanjiro's only daughter, Okatsu. He also finds out that the young yakuza working in Kanjiro's house is his younger brother Hanji (Yukio Hashi). Then the Shigezo family, who had been annoying Kanjiro for a long time, set fire to his gambling hall!

About 1786 the doings of a demented lord results in many masterless samurai, including Iyemon (Kei Sato) who is used to luxury and cannot adjust to the hand-to-mouth conditions & piecework of umbrella making. Having hired ruffians to make him look like a superior swordsman, he arranges for himself the opportunity of a profitable marriage. He hires the half-blind masseur Takuetsu (Sawamura Sounosuke) to seduce or rape his wife (Kyoko Mikage), so that she can be divorced or killed for adultery. But the masseur takes pity & informs Oiwa of her husband's horrid plot. Assisted by the merchant's daughter he intends to marry, Iyemon disfigures his wife attempting to poison her so he can marry higher. There's a lovingly gruesome sequence as she combs blad patches into her hair, kneeling deformed at her mirror, weeping with bitterness. She eventually cuts her own throat, swearing revenge.
